Tori Shiu is a pharmacist and co-founder of CareBridge Rx, a pharmacist-led medication management and care coordination service focused on improving outcomes for older adults. With experience across multiple care settings, she has seen firsthand how fragmented medication oversight contributes to preventable hospitalizations, cognitive decline, and breakdowns in care—particularly during transitions.
Tori partners with assisted living communities, home health agencies, and senior care professionals to identify high-risk medication scenarios, reduce adverse drug events, and improve communication across the care continuum. Her work focuses on bringing structure and accountability to medication management—one of the most critical yet often unowned aspects of senior care.
Her mission is both professional and personal, shaped by the loss of a family member due to gaps in care coordination, and drives her commitment to creating safer, more connected care for older adults.
Wade Shiu is a pharmacist and co-founder of CareBridge Rx, where he focuses on building partnerships and systems that improve coordination across the senior care landscape. His work centers on addressing operational gaps between providers, caregivers, and care settings—ensuring that critical aspects of care, such as medication management, do not fall through the cracks.
Wade collaborates with assisted living communities, home health agencies, and senior care professionals to implement scalable solutions that enhance communication, reduce risk, and improve overall care delivery. He brings a strategic and relationship-driven approach to solving complex challenges in senior care.
Driven by both professional experience and personal loss due to a breakdown in care coordination, Wade is committed to advancing more accountable, integrated models of care for older adults.
